If your order is lost or damaged during shipping, please fill out the following form so we can assist you. 

Orders under $100 in value are automatically insured by USPS or UPS. Orders over $100 for which additional insurance was purchased during checkout are eligible for coverage up to the amount insured at checkout. For example, if your order total was $250 and you elected for insurance up to $200, then you are eligible for only $200 in coverage.
